Yes, and it is worth calling early rather than on the day. Discharges from Sunshine, Footscray and the wider Western Health network often move faster than supports can be arranged, so the earlier we know a date is coming the more of the gap we can close before you are home.
Changing coordinator is your call and you do not need to justify it. Check the notice period in your current service agreement, tell us where things stand, and we will take over the handover — including getting the file, the provider list and whatever is half-finished.
