Support Coordination across Melbourne’s west.

Coordination works from anywhere. Knowing which physio in Werribee actually has capacity, and which provider will let you down in March, does not.

The part of coordination that cannot be done from a desk

Support Coordination is not tied to a local roster, so on paper it can be delivered from anywhere in the state. That is true and it is also slightly beside the point. The useful part of the job is knowing the specific services in front of you — which allied health practice in Sunshine has a waiting list you will never get off, which day programme in Wyndham is genuinely good, which provider says yes to everything and then quietly delivers half of it.

That knowledge is accumulated, not researched. Our team has worked the west since Gencare started, which means most of the local providers are already known quantities rather than names on a directory. It is the difference between a coordinator sending you three brochures and one telling you which of the three to call first and why.

The other thing that matters here is scale. The west is the fastest-growing part of Melbourne and its services are stretched. Getting a place often comes down to who asked, how early, and whether the request was framed the way the intake team needed to see it.

What can start now

Straight away. Support Coordination does not depend on having a worker near you, so there is no waiting list for this one — and if you are switching from another coordinator, we handle the changeover so nothing lapses in between.

On the ground here

What support coordination — level 2 in Western Melbourne actually looks like

Local providers, actually known

Not a directory search. Years of knowing which western-suburbs services deliver and which ones look better on paper than in practice.

Hospital discharge, handled

Sunshine, Footscray and Western Health discharges move fast. We work to their timelines rather than asking them to wait for ours.

Referrals that match your background

In one of Australia’s most multicultural regions, matching you to a service where you can be understood is a practical consideration, not a nicety.

A plan is a budget until somebody turns it into a week that works.

How this starts

Working it out, one step at a time

  1. Read the plan properly

Line by line, including the parts nobody explained. Most people are surprised by at least one thing in their own plan.

  2. Work out what is missing

What you are funded for, what you are actually using, and where the gap between those two is costing you.

  3. Make the calls

We approach providers, frame the request the way intake teams need to see it, and chase the ones who go quiet.

  4. Hand it back to you

Good coordination should make itself less necessary over time, not more. That is the actual goal.

Western Melbourne questions

Support Coordination — Level 2 in Western Melbourne, answered

Check your plan under Capacity Building. Support Connection is Level 1, Support Coordination is Level 2 and Specialist Support Coordination is Level 3. If you are not sure, send us the plan and we will tell you what is funded — including if you have been paying for a level you do not have.

No, and you should be suspicious of any provider who does. We coordinate the services that suit you, including ones we do not run and ones run by our competitors. If the best physio for you is somewhere else, that is where you should go — and it is your right under the NDIS to say so.

Yes, and it is worth calling early rather than on the day. Discharges from Sunshine, Footscray and the wider Western Health network often move faster than supports can be arranged, so the earlier we know a date is coming the more of the gap we can close before you are home.

Changing coordinator is your call and you do not need to justify it. Check the notice period in your current service agreement, tell us where things stand, and we will take over the handover — including getting the file, the provider list and whatever is half-finished.

The service system is the same; the knowledge of it is not. Our team has worked these suburbs for years, so most local providers are known quantities rather than names to research. In a region growing this fast, knowing who has genuine capacity right now is most of the value.
