Nursing assessment & care plan
A registered nurse assesses your needs and builds a clear clinical care plan — what care is delivered, how often, and by whom.
Skilled care at home
Your nurse delivers care in your home on a schedule that suits you, and trains and supervises support workers wherever they are involved.
Monitoring & review
We keep a skilled eye on your health, act early when something changes, and review the plan as your needs evolve — keeping everyone in the loop.
How nursing care is funded
Nursing supports can be funded through your NDIS plan where they are reasonable and necessary due to your disability. Here is the plain-English version.
Funded through: Core Supports or Capacity Building, depending on your plan and needs.
